Managing chronic conditions is a team effort that includes your doctor, nurses and specialised healthcare professionals. The goal is to help you manage your long-term health conditions better and to work with you on ways to prevent future health issues.

Support for Managing Chronic Health Conditions
Managing a long-term health condition can be complex, but you don’t have to do it alone. Medicare now offers a GP Chronic Condition Management Plan (GPCCMP) to help you coordinate your care and stay on top of your health.
A GPCCMP is a personalised plan created with your general practitioner. It outlines:

Your treatment goals

The health professionals involved in your care

The services and support you may need
This plan replaces the previous GP Management Plan (GPMP) and Team Care Arrangements (TCA), combining them into one simpler, more flexible plan.
If you have a GPCCMP, you may be eligible for Medicare-subsidised visits to allied health providers (such as physiotherapists, podiatrists, or dietitians) when the services are directly related to your plan


Creating Your Care Plan
When you book an appointment for a GPCCMP, your GP and a nurse will:

Review your health history and current needs.

Discuss your priorities and goals for managing your condition.

Identify other health professionals who may be involved in your care.

Agree on services and supports, and set a review schedule (usually every 3 months).

Provide you with a copy of your plan so you have a clear record to follow.
Appointments usually take 30–45 minutes and are designed to give you a coordinated approach to managing your long-term health needs.

Chronic conditions typically last six months or longer and include illnesses such as asthma, cardiovascular disease, diabetes, kidney disease, and musculoskeletal conditions. Discussing your specific needs with your doctor can help determine the most suitable support for you.

Patient Eligibility and Allied Health Services
​To be eligible for a GP Chronic Condition Management Plan (GPCCMP), you need to have a chronic or terminal medical condition. Your GP will talk with you about your health needs and decide if a plan is the right option for you. If you have a GPCCMP, Medicare may help cover visits to allied health providers (such as physiotherapists, podiatrists, or dietitians) when these services are part of your plan. This can include up to five individual sessions each year, plus extra group sessions for people living with type 2 diabetes. Your allied health providers will send reports back to your GP after your first and last visits, and sometimes in between. This helps keep everyone involved in your care up to date, so your plan can be adjusted if your needs change.
