Preparing for Your Appointment

To help you get the most out of your first visit, there are a couple of things to do before you come in.

New Patient Form

After you book, you’ll receive a New Patient Information Form through HotDoc. If you’d rather not do it online, you can download the form here or arrive about 15 minutes early to complete it at our clinic.

Food Diary

Before your first session, please keep a simple 3-day food diary — ideally two weekdays and one weekend day.
This gives Melissa a clear picture of your usual eating habits so your advice can be as personalised as possible.
You can download the diary here (please remember to bring it along on the day).

Consultation Fees and Rebates

If you have a Chronic Condition Management Plan or Eating Disorder Plan from your GP, you may be eligible for a Medicare rebate for dietetic services. Your GP can advise whether this applies to you.
